Adoption Finalization

This service is available to any child or youth in the legal custody of a county children and youth agency, CCYA who has a goal of adoption. Finalization services begin after the placement of a child, sibling group or an older youth into a pre-adoptive home. Payment for this unit covers the development of an Individual Permanency Service Plan, supervision of the placement by the affiliate, as well as a minimum of three supervisory reports as outlined in Attachment H of the SWAN bulletin. This applies to children or youth who are in either a foster-to-adopt or adoption situation; both statuses of families benefit from adoption specific support and supervision. Supportive services, reports and home visits, as well as work with attorneys and courts, are included in the Finalization unit. An affiliate agency can invoice for this service once the adoption is finalized (decree is filed).
